North SEERA Hockey

2010-2011 Registration Information

As an Executive we have chosen to provide registration during the summer months to alleviate the “cash crunch” we all seem to face during the busy month of September. An Early Registration fee will again be offered for those registrations received at the In-person Registration dates up-to and including July 30^th, 2010. Any registrations after that date will be at the normal fee rate, until September 1, 2010 at which point a late fee will be assessed. We have enclosed your child’s registration form for your convenience.   NO mail-in registrations will be processed, effective immediately. Check the In-person dates below for when you can register for the 2010-2011 hockey season with North SEERA.

North SEERA will no longer be running tier 1 try-outs for the 2009 – 2010 year; in the Novice or Atom Category. SEERA will be operating tier 1 try-outs for the Peewee category and will mail out to each eligible peewee player the appropriate information.

·        All transfer and new registrations MUST call Registrar for complete process instructions.

·        Registration form MUST be fully and correctly completed, updated and signed.

·        Please check the registration information carefully and MAKE any corrections, modifications or update information on the form, and return

·        Privacy statement: please read and check if you agree (left blank means you do NOT agree)

·        Include a copy of registrant’s birth certificate (new registrants or Missing on Player file)

·        Alberta Health Care Card (new registrants or Missing on Player file)

·        Parent Declaration and supporting documentation. (New or Transfer Players)

·        DO NOT PURCHASE 2010-2011 Community League memberships effective this year EFCL is having hockey programs sell corporate memberships at a rate of $25.00 per family, which will be added at the time of registration.

·        Club or SEERA Midget registration and questions should be directed to your applicable club (MLAC/SSAC) or the SEERA Hockey Association. (Check our website for links)

Payment Options:

·         Personal cheque along with any vouchers earned through North SEERA. (Cheque payable to North SEERA)

·         Visa (only) – include card #, name on card and expiration date

·         All registration fees must be paid in full at the time of registration.  (As per North SEERA parent’s Handbook page 5, #3 paragraph 2) The parent handbook is available on the North SEERA website: www.northseera.ca

DEFICIENT or Mail-In REGISTRATIONS WILL BE RETURNED UNREGISTERED

A $35.00 NSF CHARGE WILL BE APPLIED TO RETURNED CHEQUES

PLAYERS WISHING TO WITHDRAW FROM THE North SEERA HOCKEY PROGRAM MUST SUMIT A WRITTEN REQUEST TO THE REGISTRAR PRIOR TO NOVEMBER 1, 2010, TO RECEIVE A 50% REFUND

A LATE REGISTRATION FEE OF $35.00 WILL BE APPLIED TO REGISTRATIONS RECEIVED
